29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent: a) At the facility - An operator tore the right knee lateral meniscus when pushing off from a board and rising from his chair on or about May 3, 2013. This injury resulted in 30 lost work days, meeting the recording criteria of Column H on the OSHA Form 300 or its equivalent. The injury was not recorded on the OSHA 300 Form or equivalent for calendar year 2013. Abatement Certification and Documentation is required
