On February 12, 2015, next to the driveway entrance to the construction site, the spoil piles for a concrete base were at the edge of the excavation.

On February 12, 2015, next to the driveway entrance to the construction site, employees worked in an 8 foot deep by 8 foot wide by 8 foot long trench between vertical walls of soil without a protective system.
