29 CFR 1910.147(f)(4): During shift or personnel changes, specific procedures were not utilized to ensure the continuity of lockout or tagout protection, including provision for the orderly transfer of lockout or tagout devices between off-going and oncoming personnel: The employer is failing to inform second shift personnel that are engaged in Lock-out/Tag-out activities in their facility of their responsibility during shift changes. The current program does not address the requirements of this subpart.

29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose: The employer is failing to ensure that employees are protected from electrical hazards. Employees working near or on the electrical circuit breaker panel in the vacuum pump room are exposed to live parts due the front cover not being secured and in place. A screw on the panel was missing and the front panel was hanging away from the box approximately 1½" from the box allowing contact with the wiring inside the panel.
