29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose:     The employer is failing to ensure that employees are protected from electrical hazards.  Employees working near or on the electrical circuit breaker panel in the vacuum pump room are exposed to live parts due the front cover not being secured and in place.  A screw on the panel was missing and the front panel was hanging away from the box approximately 1½" from the box allowing contact with the wiring inside the panel.
