29 CFR 1910.119(e)(3)(i): The process hazard analysis did not address the hazards of the process: The employer is failing to protect employees from chemical hazards. This violation was most recently documented at a worksite located at or near 1009 W M St., Hastings, Nebraska 68901 on November 7, 2019. The 2016 PHA was performed however, the PHA that was conducted did not list or address any potential consequences of the hazard of the potential event of ammonia being vented back into the plant due to a leak from Suction 1 Control Station 37.
