29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms were not kept clean, orderly and in a sanitary condition.  On or about 12 February 2015, the spray booth used for spraying different coatings on truck beds had extensive accumulation to the point that electrical sockets, air hoses, and a ladder were difficult to identify. This exposed the employee to various safety hazards such as, but not limited to trips and falls from the sticky floor. Damage to the breathing air hose could compromise effective respiratory protection.

29 CFR 1910.134(h)(2)(i): Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals.  On or about 12 February 2015, in the spray booth area, an employee was using an airline respirator for his respiratory protection while spraying truck beds with rhino coating that contained methylene diphenyl diisocyanate (MDI). The respirator was hung on a hook located in the spray booth area where the employer did not ensure that it was protected from damage, exposing employees to an inhalation hazard.

29 CFR 1910.303(b)(2): Listed or labeled equipment was not installed or used in accordance with instructions included in the listing or labeling.  On or about 12 February 2015, in the spray booth area, a surge protective device (SPD) was being used in place of permanent wiring to power the breathing air compressor used while spray painting; exposing employees to fire and electrical hazards of ~120 volts.    Spillar Custom Hitches was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303(b)(2) which was contained in OSHA inspection number 391882 citation number 1, item number 3 and was affirmed as a final order on 6/22/12, with respect to a workplace located at 2701 S. Congress Ave, Austin TX 78704.

29 CFR 1910.1000(a)(1): Employee(s) were exposed to an airborne concentration of MDI listed in table Z-1 in excess of the ceiling concentration of 0.2 mg/m3.  On or about 18 February 2015, employees were exposed to methylene bisphenyl isocyanates (MDI) levels that were 4 (four) times the 15 minute permissible exposure limit of 0.2mg/m3. Despite wearing a supplied air respirator, the existing ventilation controls did not adequately control the isocyanates vapors to acceptable levels.   Spillar Custom Hitches was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.1000(a)(1), which was contained in OSHA inspection number 391882, citation number 1, item number 5a, and was affirmed as a final order on 6/22/12, with respect to a workplace located at 2701 S. Congress Ave, Austin TX 78704.
