29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  On or about February 6, 2015, and prior thereto, outside the lower shop building, of the Best Enterprises facility, located at 3513 West Mountain Springs road in Cabot, Arkansas,  employees manufacturing stainless steel wastewater tanks for the portable toilet industry, were operating a Bertsch and Company roll former, that was not equipped with point of operation guarding. These conditions exposed the employees to the hazard of being caught in the machine.    Best Enterprises, Inc. was previously cited for a violation of this occupational safety and health standard, which is contained in OSHA inspection number 962517, Citation 1 item 2, and was affirmed as a final order on May 29, 2014, with respect to the workplace located at 3513 West Mountain Springs Road in Cabot, Arkansas.

The employer failed to report to OSHA, the work related amputation, within twenty-four hours as required.
