29 CFR 1926.451(b)(1):  Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):  (a)Martinez Western Constructors LLC, at 1060 1st Avenue, Greeley, CO: On and before 2/12/15, Martinez Western Constructors LLC did not ensure that each platform on all working levels of scaffolds was fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii).  Employees performed work on a poured concrete foundation.  Scaffolding around the perimeter at the top of the twenty four foot tall foundation was not fully planked.  This condition exposed employees to an approximate twenty-two foot fall hazard.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was six feet (1.8m) or more above a lower level was not protected from falling by the use of a guardrail system, a safety net system, or personal fall arrest system:  (a)Martinez Western Constructors LLC, at 1060 1st Avenue, Greeley, CO: On and before 2/10/15, Martinez Western Constructors LLC did not ensure that each employee on a walking/working surface having an unprotected side or edge which was six feet (1.8m) or more above a lower level was protected from falling by the use of a guardrail system, a safety net system, or personal fall arrest system.  An employee was removing forms from a poured concrete foundation.  The employee fell approximately twenty feet to the ground when the form he had tied off to was stripped from the poured foundation.  The employee was not protected from a fall while stripping forms.  This condition exposed the employee to an approximate twenty-foot fall hazard.

29 CFR 1926.503(b)(1): The employer did not verify fall protection training by preparing a written certification record:     (a)Martinez Western Constructors LLC, at 1060 1st Avenue, Greeley, CO: On and before 2/12/15, Martinez Western Constructors LLC did not verify fall protection training by preparing a written certification record.  Employees worked at heights greater than six feet above a lower level while erecting forms, pouring concrete, and stripping forms.  The employer did not document that fall protection training was provided to the employees.
