29 CFR 1910.253(b)(4)(i): Oxygen cylinders were stored near highly combustible material, especially oil and grease; or near reserve stocks of carbide and acetylene or other fuel-gas cylinders, or near other substance likely to cause or accelerate fire;   a) Prosthetics - On or about 2/10/15 employees were exposed to explosion hazards when oxygen and acetylene cylinders were stored together.  b) AC Shop - On or about or 2/11/15 employees were exposed to explosion hazards when oxygen and acetylene cylinders were stored together.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.304(b)(2): Outlet devices had an ampere rating less than the load being served:   (a)  Carpentry Shop - On or about 2/10/15, employees were exposed to electrical hazards when a power strip was being used to provide power to equipment that exceeded the ampere rating.  The Department of Veteran affairs was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.304(b)(2), which was contained in OSHA inspection number 966924, Notice number 1, Item number 2 and was affirmed as a final order on 9/15/14 with respect to a workplace located at 1500 Weiss Street, Saginaw, MI 48602.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective:  a) Core Lab - On or about 2/11/15, employees were exposed to electrical hazards when the ground prong on the fan was missing.  b) Canteen - On or about 2/11/15, employees were exposed to electrical hazards when the ground prong on the vacuum was missing.  The Department of Veteran affairs was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.304(g)(5), which was contained in OSHA inspection number 912088, Notice number 1, Item number 2 and was affirmed as a final order on 12/12/13 with respect to a workplace located at 913 NW Garden Valley Blvd, Roseburg, OR 97470.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
