29 CFR 1926.100(a):   Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   A) Second Floor - An employee was exposed to head injuries while working below framing activities. The employee was not protected by a hard hat or protective helmet, on or about December 1st, 2014.   Estrada & Roca, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 913362, citation number 1, item number 1 and was affirmed as a final order on June 4th, 2014, with respect to a workplace located at 588 Warwic Avenue in Teaneck, NJ.

29 CFR 1926.501(b)(13):   Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b)   A) Top of structure: Employees were exposed to fall hazards of approximately 27 feet while engaged in framing work on a residential structure without a means of fall protection in place, on or about December 1st, 2014.   The company was previously cited for violations of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1926.501(b)(13), which was contained in OSHA Inspection Number  312865942, 314681214 and 913362:  In regard to Inspection Number 312865942, Citation # 1, Item # 1 was issued on 07/09/10, with respect to a work place located at 231 Churchill Road, Tenafly, NJ. This citation was affirmed as   a final order on 08/12/10.  In regard to Inspection Number 314681214, Citation # 2, Item # 1 was issued on 03/29/12, with respect to a workplace located at 209 Glen Ave. Palisades Park, NJ. This citation was affirmed as a final order on 04/16/12.
