(b)  On or about February 6, in the Clean and Strip Room, the RC-1 Crown electric forklift (Model: 30RCTT S, S/N: 1A115667) had an addition of a Valley Craft (Model:  A63331-0501, 2000 Cap) Ultra-Grip clamp to the forks of the forklift without manufacturer's prior written approval.  (c)  On or about February 6, in the Clean and Strip Room, the Valley Craft (Model:  A6331-0102) Ultra-Grip clamp was added to a sit down electric forklift without the manufacturer's prior written approval.

(b)  On or about February 6, in the Clean and Strip Room, the nameplate on the RC-1 Crown electric forklift (Model: 30RCTT S, S/N: 1A115667) was not up-dated to include the Valley Craft (Model:  A63331-0501, 2000 Cap) Ultra-Grip clamp.   (c)  On or about February 6, in the Clean and Strip Room, the nameplate for the sit down forklift was not up-dated to include the Valley Craft (Model:  A6331-0102) Ultra-Grip clamp.
