29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:  (a)  In the Clean and Strip Room, the RC-2 Crown electric forklift (RC 3000 Series, S/N:  1A289707) had been modified with the Cascade clamp (Model 35D Revolving Clamp) without manufacturer's prior written approval.  (b)  On or about February 6, in the Clean and Strip Room, the RC-1 Crown electric forklift (Model: 30RCTT S, S/N: 1A115667) had an addition of a Valley Craft (Model:  A63331-0501, 2000 Cap) Ultra-Grip clamp to the forks of the forklift without manufacturer's prior written approval.  (c)  On or about February 6, in the Clean and Strip Room, the Valley Craft (Model:  A6331-0102) Ultra-Grip clamp was added to a sit down electric forklift without the manufacturer's prior written approval.

29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered:   (a)  In the Clean and Strip Room, the nameplate on the RC-2 Crown electric forklift (RC 3000 Series, S/N:  1A289707) was not up-dated to include the Cascade clamp (Model 35D Revolving Clamp) which was installed after the factory installed a Cascade Sideshifter 55F-SSS-AO62.   (b)  On or about February 6, in the Clean and Strip Room, the nameplate on the RC-1 Crown electric forklift (Model: 30RCTT S, S/N: 1A115667) was not up-dated to include the Valley Craft (Model:  A63331-0501, 2000 Cap) Ultra-Grip clamp.   (c)  On or about February 6, in the Clean and Strip Room, the nameplate for the sit down forklift was not up-dated to include the Valley Craft (Model:  A6331-0102) Ultra-Grip clamp.
