29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level. a). North Interior Wall - On or about February 5, 2015, personal fall protection was not rigged to prevent contact with a metal grated catwalk that was located below an employee's working surface, thus exposing the employee to a fall hazard of approximately 6 feet 3 inches onto metal grating.
