29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, and rotating parts. Location - Polishing Machine: On or about February 13, 2015, employees were exposed to hazards created by ingoing nip points and rotating parts in that the stationary arms and the ingoing nip points created by the rotating disks on the Lapmaster LM84 were not effectively guarded.

29 CFR 1910.212(a)(3)(iii): Special hand tools for placing and removing material(s) was not provided to permit easy handling of material without the operator placing a hand in the danger zone: Location - Polishing Machine: On or about February 13, 2015, employees were exposed to hazards at the point of operation on the Lapmaster LM84 in that they were not effectively protected from the hazard created by the spinning surface while cleaning the polishing machine table.
