29 CFR 1910.134(f)(2): Employee(s) using a tight-fitting facepiece respirator were not annually fit tested: (a) Main Building: The employer did not provide a fit test for an employee who was required to wear a half face respirator such as a Survivair Sperion brand Half Face Air Purifying Respirator with organic vapor cartridges for respiratory protection while conducting spray painting operations using, but not limited to, Rust-oleum Pro 1- GL Gloss White paint which contains Ethylbenzene, and mixing Kaocrete which contains silica, as determined on or about 10/08/2015. ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.134(g)(1)(i)(A): Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function: (a) Main Building: An employee was required to wear a half face respirator such as a Survivair Sperion brand Half Face Air Purifying Respirator with organic vapor cartridges for respiratory protection while conducting spray painting operations using, but not limited to, Rust-oleum Pro 1- GL Gloss White paint which contains Ethylbenzene. The employees using the respirators had facial hair that came between the sealing surface of the facepiece, as determined on or about 10/08/2015. ABATEMENT CERTIFICATION REQUIRED
