Samuel Stamping Technologies LLC - On or about, 2/17/15, employees conducted servicing and maintenance on machines/equipment that included but was not limited to mechanical and hydraulic presses. The specific lockout/tagout procedures were missing key elements, including, but not limited to, magnitudes of energy and the use of devices to restrain stored energy. Sources of stored energy included but were not limited to the ram and upper tooling platens on the mechanical and hydraulic presses.
