29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by in-going nip points:  A. On, or about, February 6, 2015, an employee suffered serious injuries when his hand was pulled into the in-running nip point of a chain and sprocket while diagnosing a machine malfunction on #3 Cook Machine, an Entwistle Model SO-24V Single Twist Cabler, serial number 2003 L-R.  At that time of the accident, the traverse limit switch was positioned under the chain and sprocket drive and the lettering on the operational mode selector switch was faded.  B.  On, or about, February 27, 2015, a green Entwistle Single Twist Cabler located across the aisle from #3 Cook Machine had the traverse limit switch located in close proximity to the chain and sprocket drive.

29 CFR 1910.332(b)(1): Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments:  Company:  On, or about, February 27, 2015, maintenance personnel had not been trained regarding what work practices and protective equipment was required to protect them against electric shock, arc flash and arc blast hazards while performing live electrical work, live trouble-shooting and verifying whether or not electrical equipment was properly de-energized.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not using electrical protective equipment that was appropriate for the specific parts of the body to be protected and for the work to be performed:  Company:  On, or about, February 27, 2015, personal protective equipment such as, but not limited to, voltage rated gloves, long sleeve fire resistive clothing, face shields, balaclavas, hard hats and hearing protection were not routinely worn by employees who performed live electrical work, including trouble-shooting, or who verified that electrical equipment was properly de-energized.

29 CFR 1904.39(a)(2): The employer did not report the in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours of the incident:    On or about February 6, 2015, an employee suffered a finger amputation while working on a single twist cabling machine.  The incident was reported to OSHA on February 11, 2015.
