On or about February 18, 2015, the employer failed to provide adequate guarding in that, employees working on or around the rainbow cooling table were exposed to struck-by hazards when product that reached the top of the table would free roll down an incline on the back side and into a catch collection area.

Employees worked in this area without the use of appropriate eye and face protection that would protect against liquid chemicals entering the eyes or coming into contact with the face.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use. a) On or about February 18, 2015 employees working in the pickling operations were exposed to sulfuric acid without suitable facilities for the quick drenching of the eyes and body, in that the eye wash located at building 22 near the 23 West marker was found to be inoperable due to it being frozen.
