29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly.    a. In the warehouse located in Building H at 2249 Westbrook Drive, Columbus, Ohio, employees used a Vestil Telescoping Boom forklift attachment to hoist single stone slabs. The attachment was not secured to the forklift to prevent displacement and/or shifting. The data plate on the two CAT GC70K Forklifts, serial AT89B00192 & AT89A10349, did not show the rated capacity of the forklift with the attachment. The condition affected the capacity and safe operation of the lifts.     b. In the warehouse located in Building H at 2249 Westbrook Dr., Columbus, Ohio, employees used a modified ASB 106M2 Abaco Spreader Bar to load trucks and place bundles of stone slabs on a-frames in the warehouse. The spreader bar was modified with forklift pockets and was used without securing the attachment to the forklift. The modification was not approved by the spreader manufacturer and the data plate on the two CAT GC70K Forklifts, serial AT89B00192 & AT89A10349, did not show the rated capacity of the forklift with the attachment. The condition affected the capacity and safe operation of the forklift.      c. In the warehouse located in Building H at 2249 Westbrook Dr., Columbus, Ohio, employees used a CAT GC70K forklift, serial AT89A10349, with inverted forks. The data plate did not show the change in capacity due to the modification of inverted forks. The modification affected the capacity and safe operation of the forklift.

In the warehouse located in Building H at 2249 Westbrook Dr., Columbus, Ohio, employees were exposed to a crushed-by hazard while removing bundles of stone slabs from storage shipping containers. Employees were reaching between bundles to perform the job task without ensuring that the bundles were secured to prevent displacement.   Stone Mart was previously cited for a violation of this occupational safety and health standard or its equivalent standard OSH ACT of 1970 Section (5)(a)(1) which was contained in OSHA inspection number 948159, citation number 1, item number 1 and was affirmed as a final order on February 13, 2014 , with respect to a workplace located at 2249 Westbrook Drive in Columbus, Ohio.
