29 CFR 1915.89(n)(2): The employer did not ensure that each lock and tag was uniquely identified for the purpose of controlling hazardous energy and was not used for any other purpose. F/T American triumph, Kloosterboer Dock, Baader room: On or about 5 February 2014 and times prior thereto, a danger tag was affixed to an operating refrigerator. This was not in accordance with the administrative procedures of the company Lock-out/Tag-plus system.
