Hazards include but are not limited to roofers working without fall protection, improper use of ladders, employees and/or sub-contractors utilizing aerial lifts and not utilizing fall protection equipment and/or not using it correctly. 29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:    (a) 2000 W John Loop, Coeur d' Alene, ID:  On or about February 11, 2015, an employee without a personal fall arrest system was observed working in a aerial lift and it was extended approximately 17 feet above ground.     (b) 2000 W. John Loop, Coeur d' Alene, ID:  On or about February 11, 2015, an employee working with a Personal Fall Arrest System (PFAS) attached to a strap anchor and anchored to the top guard rail of the aerial lift and extended approximately 17 feet above ground.      Note (1): Abatement certification IS required for this item.    Note (2): Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part 1926.502(d)) that provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e).

29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    a) 2000 W John Loop, Coeur d'Alene, Idaho: On or about February 11, 2015, the controlling employer, Edwards-Smith Construction, LLC did not ensure that its sub-contractor Granite Enterprises, Inc., protected their employees while performing roofing activities.  Employees were exposed to fall hazards ranging from 12 foot up to heights of 25 feet while working on this 18 in 12 pitch structure.     Note) Abatement certification IS NOT required for this item.
