On or about 02/13/2015, at machine two, white belt conveyor - an employee applying grease to a chain and sprocket wheel assembly while the machine was running, had the tip of the right hand index finger amputated. The guard was removed and lockout / tagout procedures were not used.

Employees are required to remove jammed product in that area to keep the equipment running and production moving.
