29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Employees engaged in maintenance and production activities at the following locations were exposed to amputation and struck-by hazards in that machine guarding was not provided: a) On or about February 3, 2015, an employee engaged in production activities at the Spiral #1 conveyor was exposed to caught-between hazards in that the in-running nip point was not guarded. b) Employees engaged in maintenance activities using the Jett drill press in the maintenance shop were exposed to struck-by hazards in that machine guarding was not provided.

29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees: Employees engaged in maintenance activities in the center of the maintenance shop were exposed to electrical shock hazards in that the screw fitting which connected the flexible cord to the junction box had become disconnected exposing the conductors.
