29 CFR 1915.89(c)(1):   The employer did not ensure the use of a lock to prevent energization or startup, or the release of hazardous energy, before any servicing was started.    F/V Cipper Epic, Coastal dock, Factory area:  On or about 7 February 2015 and times prior thereto, the two conveyor belts of the had tank were removed and the hydraulic power system was not isolated out.  This condition exposed employees to a caught in hazard.      Clipper Seafoods was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 192153, citation number 1, item number 1 and was affirmed as a final order on April 20, 2012, with respect to a workplace located at Dutch Harbor.
