29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.    A workers was exposed to fall hazards while working from an aluminum pump jack scaffold greater than 10 ft from the ground below with no form of fall protection and no alternative form of fall protection in use.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: (a)      Workers were exposed to fall hazards greater than 10 feet while performing roofing and framing activities without fall protection. The workers  were observed wearing a full body harness but they were not connected to an anchor point.  No alternative from of fall protection was observed at the work site.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    Workers were exposed to fall hazards greater than 10 feet 9 inches from the concrete surface below while using a 6 foot A-frame step ladder to access the upper working surface.  The ladder did not extend at least 3 feet above the working surface landing.
