29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used: This violation occurred on or about February 20, 2015 at a jobsite located at 660 Terrace Street in Coppell, Texas; where at least three (3) employees were not provided with a portable or hook on ladder and were using the end frames and cross braces to gain access to the upper levels of the two tier tubular welded frame scaffold.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level: This violation occurred on or about February 20, 2015 at a jobsite located at 660 Terrace Street in Coppell, Texas; where at least one (1) employee was performing masonry brickwork and was not protected from falling more than 13 feet to the ground level below.
