On or about February 20, 2015 an employee of Roeder Construction was observed installing vinyl siding while standing on a step ladder platform scaffold. The platform of the scaffold was placed on the highest rung/step of the ladder on the east side of the residence and the second highest rung/step of the ladder on the west.

Roeder Construction jobsite located at 500 May Street, Gibbon, NE: The employer failed to provide training for employees working on and/ or with scaffolds on the hazards associated and procedures used to control / minimize the hazards. On or about March 17, 2015 an employee of Roeder Construction was observed installing vinyl siding from a ladder plank scaffold. The scaffold was not erected properly in that the plank was higher by approximately 10 inches on one side. The plank was also placed on the top rung of the step ladder on the low side.
