29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied: Worksite located at 10700 NW 4th. Yukon, OK 73099. On or about February 19, 2015, a fork-lift was used to lift an employee working from a man-basket while the basket was not attached securely to the fork-lift. The employee was exposed to a fall hazards of approximately 14 feet.
