29 CFR 1910.95(d)(1): When information indicated that any employee's exposure equaled or exceed the 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program:  Freezer hold:  On or about 2/4/2015, and times prior thereto, employees working in the freezer hold unloading boxes of frozen fish were exposed to noise at between 97.5 dB(A) and 105 dB(A) for 2 hours.  None of the employees were wearing hearing protectors and were exposed to noise hazardous conditions.
