29 CFR 1910.95(d)(1): When information indicated that any employee's exposure equaled or exceed the 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program: Freezer hold: On or about 2/4/2015, and times prior thereto, employees working in the freezer hold unloading boxes of frozen fish were exposed to noise at between 97.5 dB(A) and 105 dB(A) for 2 hours. None of the employees were wearing hearing protectors and were exposed to noise hazardous conditions.

29 CFR 1910.1200(f)(6)(i): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the information required by 29 CFR 1910.1200(f)(1)(i) through 29 CFR 1910.1200(f)(1)(v): (a) Case up area: On or about 2/4/2015, and times prior thereto, the chlorinator container system was not labeled where employees add a corrosive chlorinated solution into a tank; (b) Factory Area: On or about 2/4/2015, and times prior thereto, an iodine container used for sanitization was not labeled in the factory; (c) Factory Area: On or about 2/4/2015, and times prior thereto, ammonia piping lines in the factory were not labeled; exposing employees to chemical hazards in the workplace.
