This exposed employees to struck-by hazard from work piece getting stuck between rest and wheel causing wheel to break.

This exposed employees to struck-by hazards if the wheel broke.

Engine room:  On or about February 7, 2015 and times prior thereto, an air nozzle used for cleaning was operating at 90 p.s.i.  This exposed employees to laceration/air embolism hazard from using an air nozzle that would not reduce if dead-ended with skin.

This exposed employees to accelerated fire hazard if the bottles were exposed to a fire.

This exposed employees to struck-by hazard on the weather deck to mechanical failures of the crane.
