29 CFR 1917.95(c): The employer did not provide well maintained emergency bathing, eye washing or other facilities where employees were exposed to hazardous substances. OSI Dock, Maintenance shop: On or about 12 February 2015 and times prior thereto, the emergency eyewash station was blocked with a supplies and a bicycle. This condition exposed delays in employees to utilize the eyewash.

29 CFR 1910.95(d)(1): When information indicated that any employee's exposure equaled or exceed the 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program: OSI Dock, Freezer : On or about 12 February 2015 and times prior thereto, hearing conservation monitoring had not been completed for the freezer. This condition allowed for employees to not have the knowledge of noise hazardous conditions and where to wear hearing protection while in the freezer.

29 CFR 1910.1200(f)(6)(i): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the information required by 29 CFR 1910.1200(f)(1)(i) through 29 CFR 1910.1200(f)(1)(v): OSI Dock Maintenance shop: On or about 12 February 2015, and times prior thereto, there was an unlabeled container in the work area. This exposed employees to delays in getting proper first aid and knowing the correct PPE for the type of chemical.
