On February 24, 2015, 42 circuit breakers in the main electrical panel were not marked to indicate its purpose. The circuits had supplied power to various lights and outlets located throughout the project and there were multiple electrical boxes with no covers that had wires connected to the breakers.  There were multiple metallic sheathed cables, with protruding wires that were connected to the breakers, hanging from electrical boxes, exposing employees to the hazard of contacting electricity.

Equipment or circuits that were deenergized were not rendered inoperative and did not have tags attached at all points where such equipment or circuits can be energized:        a.  On February 24, 2015,  38 of the 42 circuit breakers in the main 220 VAC electrical panel were in the off position.  Two breakers were in the off position and not covered with masking tape that indicated they should remain off.  Thirty-six breakers were covered with masking tape that indicated they should remain off. There were multiple electrical boxes with no covers that had wires connected to the breakers.  There were multiple metallic sheathed cables, with protruding wires that were connected to the breakers, hanging from electrical boxes, exposing employees  to the hazard of contacting electricity.
