29 CFR 1926.102(a)(2): Eye and face protection equipment required by this Part did not meet the requirements specified in American National Standards Institute, Z87.1-1968, Practice for Occupational and Educational Eye and Face Protection:   a)  On February 24, 2015, eye protection was ordinary sunglasses not Z87 approved, exposing employees to the hazard of being struck by concrete debris, steel studs, rebar and other debris being torn out of the structure.

29 CFR 1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structural members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high:  a)  On February 24, 2015, at the backside of the building on the third level, there was no midrail or equivalent for about 12 feet along the area where the debris slide was located, exposing employees to the hazard of falling about 9 feet onto the debris pile or about 18 feet to the asphalt below.

29 CFR 1926.852(b): All materials chutes, or sections thereof, at an angle of more than 45 deg. from the horizontal, were not entirely enclosed, except for openings equipped with closures at or about floor level for the insertion of materials:  a)  On February 24, 2015, on the backside of the building, a material chute at an angle of about 62 degrees to the horizontal from the third level floor to the ground was not enclosed, exposing employees on lower to the hazard of being struck by debris being dumped on the slide from the third floor.
