29 CFR 1926.651(i)(1): Where the stability of adjoining buildings, walls, or other structures was endangered by excavation operations, support systems such as shoring, bracing or underpinning was not provided to ensure the stability of such structures for the protection of employees: a) Employees were exposed to cave-in and struck-by hazards while required to erect formwork without support systems being provided to ensure the stability of a building and other structures adjacent to a vertical excavation wall having a depth of approximately 13 feet along the north side of the foundation excavation located at 900 N. Vail Street, Arlington Heights, Illinois. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation; item f (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

Vail Street, Arlington Heights, Illinois. b) Employees were exposed to cave-in and struck-by hazards while required to erect formwork without being protected from cave-ins by adequate shoring, sloping, or other protective means when working between a vertical excavation wall and formwork at a depth of approximately 9 feet along the east side of the foundation excavation located at 900 N. Vail Street, Arlington Heights, Illinois.
