29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section.  On 2/18/15, at the Suwanee location, the employer did not create and implement a specific written procedure for the extrusion line where "lip jam removals" occurred.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.   On 2/18/15, the employer did not ensure a periodic inspection was conducted and included all Authorized employees on an at least annual basis. The previous inspection expired on 2/17/14.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

On 2/18/15, at the Suwanee job site, employees performing "Authorized" service/maintenance work at the extrusion line (i.e. "lip wrap jam removal") were not trained as "Authorized employees".    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1910.1200(h)(3)(iv):   The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information.  On 2/18/15, at the Suwanee job site, the employer did not ensure employees (including Hire Dynamics temporary employees) were provided Global Harmonization System training to make employees aware of new label requirements for chemicals. Chemical employees may be exposed to include: Talc, HM10 Max, and AW-2443 Opaque White.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
