29 CFR 1926.502(b)(13): When guardrail systems were used around holes which were used as access (such as ladderways), they were not provided with a gate, or were not so offset that a person could not walk directly into the hole: On or about February 25, 2015, during the construction of a new residential home, the employer did not ensure employees were protected from fall hazards of approximately 20 feet by utilizing a fall protection system/device during the installation of wooden rafters and roof sheeting material.
