29 CFR 1910.1025(d)(6)(ii): Where the initial determination or subsequent monitoring revealed employee exposure to lead to be at or above the action level but below the permissible exposure limit; monitoring was discontinued without two consecutive measurements taken at least 7 days apart below the action level: (a) Piezo Kinetics, Inc. - Air monitoring conducted by the employer for the Grinding operation revealed an 8 hour Time Weighted Average (TWA) lead exposure of 33 ug/M3 on January 16, 2014. Air monitoring conducted by the employer on May 1, 2014 revealed an 8 hour TWA lead exposure of 24 ug/M3. The employer did not repeat air monitoring at least once every six months thereafter until at least two consecutive measurements, taken at least 7 days apart were below the action level, on or about February 26, 2015. No abatement certification or documentation required.

29 CFR 1910.1025(n)(1)(ii)(D): The employer's exposure monitoring record did not include the names or social security numbers of all other employees whose exposure the measurement was intended to represent: (a) Piezo Kinetics, Inc. - The employer's exposure monitoring records for the Pressing and Stacking operations did not include the names of all of the other employees whose exposure the measurements represented, on or about February 26, 2015. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
