29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk were not guarded by either a standard railing with standard toeboard on all exposed sides, or a floor hole cover of standard strength and construction. a) Jobsite: Ring crusher; along railing under in-feed conveyor - Employees conducting recycling activities were exposed to trip and fall hazards from a floor hole missing a guard and/or cover. The floor hole measured 16 x 8 inches. b) Jobsite: Ring crusher; where flexible electrical conduits run through adjacent to the eject door - Employees conducting recycling activities were exposed to trip and fall hazards from a floor hole missing a guard and/or cover. The floor hole measured 13 x 15 1/2 inches. Observed on or about 2/20/15

29 CFR 1910.23(e)(1): A standard railing shall consist of top rail, intermediate rail, and posts, and shall have a vertical height of 42 inches nominal from upper surface of top rail to floor, platform, runway, or ramp level. The top rail shall be smooth-surfaced throughout the length of the railing. The intermediate rail shall be approximately halfway between the top rail and the floor, platform, runway, or ramp. The ends of the rails shall not overhang the terminal posts except where such overhang does not constitute a projection hazard. a) Jobsite: Ring crusher, front and rear staircase - Employees were conducting recycling operations using stairways with railings missing midrails. b) Jobsite: Ring crusher, under in-feed conveyor - Employees were conducting recycling operations on a platform, approximately 10 feet above ground level, without standard railings. c) Jobsite: Ring crusher, behind anvil motor - Employees were conducting recycling operations on a platform with a broken railing. Observed on or about 2/20/15
