29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about February 26, 2015 at the jobsite located at or near Two Plaza drive in Liberty MO employees were exposed to a fall hazard in that they were working on a steep roof approximately 20 feet above ground level without the use of approved form of fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about February 26, 2015 at the jobsite located at or near Two Plaza drive in Liberty, MO employees were exposed to a fall hazard in that employees were accessing a working surface on a steep pitch roof by way of a portable ladder whose side rails did not extend at least 3 feet above the upper landing surface being accessed.
