29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. Worksite 16264 Rolling Ridge Road, Omaha, NE 68135: An extension ladder used to access a roof surface didnt have the side rails extended at least 3 feet above the upper landing surface (roof). Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citations.

29 CFR 1926.1060(b): The employer did not provide retraining as necessary for each employee using ladders and stairways so that the employee maintained the understanding and knowledge acquired through prior training: Worksite 16264 Rolling Ridge Road, Omaha, NE 68135: A 20 foot aluminum extension ladder was not being used correctly at the site. Training with the employees had been conducted by the company owner; however, the information was not correct. The employer also failed to provide any necessary retraining to ensure that the employee maintained the knowledge. Abatement certification is required for this violation. The abatement certification sheet is enclosed with the citations.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): Worksite 16264 Rolling Ridge Road, Omaha, NE 68135: Employees were observed working on steep residential roof (greater than 4 in 12 pitch) performing roofing activities without any type of fall protection system in use. This exposed the employees to falls ranging in height from 9 to 19 feet. Stone Coated Steel Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number1038387, Citation #1, Item #3, and became a final order on March 20, 2015. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citations.
