29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary.  On or about February 27, 2015 and at times prior thereto, an employee on a platform not fully planked on a fabricated frame scaffold, approximately 13 feet above the ground, was exposed to fall hazards.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1926.451(e)(5)(i): Ramps and walkways 6 feet (1.8 m) or more above lower levels did not have guardrail systems that comply with subpart M of this part -- Fall Protection.  On or about February 27, 2015 and at times prior thereto, an employee walked on a wood ramp between the metal fabricated frame scaffold, 13' above the ground and the roof of the Old Georgetown Theater without fall protection.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.  On or about February 27, 2015 and at times prior thereto, an employee was working on a metal fabricated frame scaffold, 13 feet above the ground below, without fall protection.      Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.
