29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary. On or about February 27, 2015 and at times prior thereto, an employee on a platform not fully planked on a fabricated frame scaffold, approximately 13 feet above the ground, was exposed to fall hazards. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.

On or about February 27, 2015 and at times prior thereto, an employee walked on a wood ramp between the metal fabricated frame scaffold, 13' above the ground and the roof of the Old Georgetown Theater without fall protection. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.
