29 CFR 1910.23(c)(3): Regardless of height, open-sided floors, walkways, platforms, or runways above or adjacent to dangerous equipment, pickling or galvanizing tanks, degreasing units, and similar hazards shall be guarded with a standard railing and toe board. Production area - On or about February 23, 2015, employees used a chair as an elevated platform to transfer dough from the drum mixer into the moving turntable. The chair was approximately 16 5/8 inches above ground level. The platform (chair) was not guarded with standard railing and a toe board. Employee(s) were thereby exposed to the hazards associated with falls into a moving turntable with in-running rollers.

The certification shall identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection.
