29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: A.) On or about Wednesday, February 25, 2015 at the D261344 Kew Gardens Interchange Expansion Project The employer failed to ensure an operator of a dump truck was in a safe location during the loading of concrete debris into a haulage truck resulting in an employee sustaining injuries to the left leg after being struck by the truck's tailgate lever. ABATEMENT DOCUMENTATION IS REQUIRED PURSUANT TO 29 CFR 1903.19
