29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head: On or about 3/2/2015, at 426 Devon Brook Court, Lot 29A, Lawrenceville, Georgia: where two employees performing felt and shingle roofing installation were exposed to fall hazards of up to 23 feet to the lower level where they wore both a full body harness and were connected to the roof with a rock climbing harness.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. On or about 3/2/2015, at 426 Devon Brook Court, Lot 29A, Lawrenceville, Georgia: where two employees performing felt and shingle roofing installation were exposed to struck-by hazards from nails, and shingles, when they used nail guns without wearing eye protection. FAMA Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 950108, citation number 1, item number 1 and was affirmed as a final order on 7/16/2014, with respect to a workplace located at 5659 Sunbury Loop, Grovetown, GA.
