29 CFR 1926.102(a)(1): The employer does not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  a. Employees using nail guns while performing roofing work are not being protected from struck by hazards to their eyes through the use of eye protection:   The employer fails to assure effective eye protection against struck by hazards as required by the standard, as demonstrated on December 9, 2016, at Lot 8, 1222 Park Hollow Lane, Lawrenceville, Georgia, when four employees were not wearing eye protection while performing roofing work with nail guns, which exposed their eyes to struck-by hazards from wood chips and nails.  Fama Construction, LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1043716, Repeat citation number 2, item number 1, affirmed as a final order on April 19, 2015, with respect to a workplace located at 426 Devon Brook Ct., Lawrenceville, Georgia.   Fama Construction, LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1007529, Repeat citation number 1, item number 1, affirmed as a final order on January 21, 2015, with respect to a workplace located at 308 Brittney Cove, Lot 182, Loganville, Georgia. Employees performing roofing work at elevations of six feet or more above the ground are not being protected from fall hazards by a fall protection system:   The employer fails to assure effective protection against falls as required by the standard, as demonstrated on December 9, 2016, at Lot 8, 1222 Park Hollow Lane, Lawrenceville, Georgia, when four employees installing asphalt shingles on a 10/12 slope roof failed to attach a lifeline to their harnesses and were exposed to a fall hazards of approximately 11.1 feet to 25 feet or greater.    Fama Construction, LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1007529, Repeat citation number 1, item number 2, affirmed as a final order on January 21, 2015, with respect to a workplace located at 308 Brittany Cove, Loganville, Georgia. 29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about December 9, 2016, at Lot 8, 1222 Park Hollow Lane, Lawrenceville, Georgia:  Four employees performing roofing work were exposed to a fall hazard of approximately 11.1-feet when they used an aluminum extension ladder that was not extended three feet above the roof landing.
