29 CFR 1910.22(b)(1): Where mechanical handling equipment is used, sufficient safe clearances shall be allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made. Aisles and passageways shall be kept clear and in good repairs, with no obstruction across or in aisles that could create a hazard. a) On or about March 5, 2015, in the hazardous material storage area located in Building 479, the entrance door metal threshold was broken and protruding upwards resulting in an obstruction across the doorway. Employees were exposed to slip/trip hazards which could result in cuts, scrapes, abrasions and broken bones.

29 CFR 1910.26(c)(2)(vii): Ladders having defects are to be marked and taken out of service until repaired by either maintenance department or the manufacturer. a) On or about March 5, 2015, in Building 41 at column 31e, a metal platform ladder was defective in that the bottom support rail was bent and was not taken out of service. Employees were exposed to a fall hazard which could result in fractures, broken bones and possible death.

29 CFR 1910.213(h)(4): Radial Saws: Installation shall be in such a manner that the front end of the unit will be slightly higher than the rear, so as to cause the cutting head to return gently to the starting position when released by the operator. a) On or about March 5, 2015, in the woodworking shop located in Building 40 , the radial arm saw cutting head would not return gently to the starting position when released by the operator. Employees were exposed to cuts, scrapes and possible amputations of body parts.

29 CFR 1910.305(g)(1)(iv)(A): Flexible Cords, unless specifically permitted, may not be used: As a substitute for the fixed wiring of a structure. a) On or about March 5, 2015, at the packing station in Building 41, a flexible cord was being used as permanent wiring. Employees were exposed to electrical hazards which could results in burns, shock and possible electrocution.
