The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.    a) On or about March 3, 2015, the company did not have a written lockout program that included procedures, training and periodic inspections on how and when to isolate energy (gravity) while performing maintenance and/or cleaning of equipment such as but not limited to the Bobcat T750 Skid Steer s/n ATF613041, in that the bucket was elevated and lockout was not applied.
