29 CFR 1910.219(f)(1): Gear(s) were not guarded by a complete enclosure or by one of the methods specified in 29 CFR 1910.219(f)(1)(ii) and (f)(1)(iii).    The employer does not guard the gear on a machine whose operation exposes employees to injury. This violation was observed on or about March 13, 2015 in the machine shop area, on the south wall, where an employee was exposed to a struck-by and caught-in-between hazard when operating a roll bender machine without a complete enclosure to guard the gear.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that gear(s) are guarded by a complete enclosure or by one of the methods specified in 29 CFR 1910.219(f)(1)(ii) and (f)(1)(iii).

29 CFR 1910.252(b)(2)(iii): Workers and other persons adjacent to the welding area were not protected from the rays by noncombustible or flameproof screens or shields.    The employer does not ensure that workers and other persons adjacent to the welding area are protected from the rays by noncombustible or flameproof screens or shields. This violation was observed on or about March 13, 2015, in the fabrication area, located in the northeast area of the building, where employees and other persons were exposed to an arc flash burn hazard when welding without a flameproof screens or shields.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all workers and other persons adjacent to the welding area are protected from the rays by noncombustible or flameproof screens or shields.

The employer does not guard the point of operation on machines whose operation exposes employees to injury.      a. This violation was observed on or about March 13, 2015 in the shop area, in the southwest area near the roll up door where an employee was exposed to a struck-by and or caught-in-between hazard when operating a Shear Press (brand) press break, model  725014, serial number 4745, a without point of operation guard.    b. This violation was observed on or about March 13, 2015 in the shop area, in the southwest area near the roll up door where an employee was exposed to a struck-by and or caught-in-between hazard when operating an Accushear (Model 837512) brand shear without a point of operation guard.    c. This violation was observed on or about March 13, 2015 in the shop area, in the southwest area near the roll up door where an employee was exposed to a caught-in-between hazard when a roll bender (unknown brand) a without point of operation guard.    d. This violation was observed on or about March 13, 2015 in the shop area, on the north wall where an employee was exposed to a struck-by and or caught-in-between hazard when operating an Uni-Hydro Ironworker, model 120-24, serial number 3P12099X, without a point of operation guard.    Greenwood Manufacturing Inc. dba Tech Fab was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 315130815, citation number 1, item number 2, and was affirmed as a final order on March 1, 2011, with respect to a workplace located at 7450 Miller Road 2, Houston, TX 77049.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all the point of operations on all machines are guarded.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws.  The employer does not ensure that all flexible cords are connected to fittings so that tension would not be transmitted to joints or terminal screws.     a. This violation occurred on or about March 13, 2015, on the southwest area of the shop near the Accushear, where an employee was using a fan and radio without strain relief between the flexible cord and receptacle.   b. This violation occurred on or about March 13, 2015, on the east wall of the shop, where employees use a Miller Dimension NT 450 (unit #12), serial number LG220796C, welding power source without strain relief between the flexible cord and the receptacle.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that strain relief is provided on all flexible cords and cables.
