29 CFR 1910.22(c): Covers and/or guardrails were not provided to protect personnel from the hazards of open pits, tanks, vats, ditches, etc.    a) On or about March 4, 2015, the employer did not provide covering or other fall protection for the three oil change pits when not in use and the uncovered portions of pit openings during work on a shorter vehicle.  Employees were exposed to falls over five feet onto the steel mesh surface below.

29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:    a) On or about March 4, 2015, the employer did not ensure that employees working at the facility located at 2757 Calumet Drive Sheboygan, Wisconsin were using bump caps, burn sleeves, or safety glasses, as needed, while performing automotive maintenance activities.  Employees are not wearing safety glasses while using corrosive floor cleaning products.

29 CFR 1910.1200(f)(5): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged, or marked with the identity and appropriate hazard warnings regarding the chemicals contained therein:    (a) On or about March 4, 2015, in the lower level of the facility located at 2757 Calumet Drive Sheboygan, Wisconsin, a spray bottle containing window cleaner was not marked with the identity of the contents or the hazards associated with the liquid which the employees used on a daily basis.    b) On or about March 4, 2015, in the lower level of the facility located at 2757 Calumet Drive Sheboygan, Wisconsin, a tote containing engine coolant was not marked with the identity of the contents or the hazards associated with the liquid which the employees used on a daily basis.    c) On or about March 4, 2015, in the upper level of the facility located at 2757 Calumet Drive Sheboygan, Wisconsin, a five gallon container containing engine coolant was not marked with the identity of the contents or the hazards associated with the liquid which the employees used on a daily basis.
