29 CFR 1910.138(a) Employers did not select and require employees to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasions; punctures; chemical burns; thermal burns; and harmful temperature extremes; For forklift operators who changed out propane cylinders approximately daily without wearing chemical resistant gloves.

29 CFR 1904.10(a): When an employee's hearing test (audiogram) revealed that the employee had experienced a work-related Standard Threshold Shift (STS) in hearing in one or both ears, and the employee's total hearing level was 25 decibels (dB) or more above audiometric zero (averaged at 2000, 3000, and 4000 Hz) in the same ear(s) as the STS, the case was not recorded on the OSHA 300 Log: For the two employees with recordable Standard Threshold Shifts (STS) in 2014 who were not retested and not recorded on the 2014 OSHA 300 Log.
