29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: 120 Washington St., Worcester MA On 3/4/15 and at times prior, employees lifted a hinged guard, while the machine was operating, in order to remove jammed dough from a pie crust hole-cutting machine. On 2/26/15, an employee received a finger amputation injury clearing a dough jam with the hinged guard opened.
