29 CFR 1910.305(b)(2)(i): Covers of outlet boxes having holes through which flexible cord pendants passed were not provided with bushings designed for the purpose nor did the holes have smooth, well-rounded surfaces on which the cords could bear:    On or about March 03, 2015 and at times prior thereto, employees were working on a rig site, Cactus Drilling Rig #163. Trailer #224 where the company man was housed had a high voltage 240 volt box which lacked a proper bushing that caused the cable to be strained. This exposed employees to electrical shock hazards.
